Once you have your machinepicked out and your sponsor info
you are ready to print the
following forms, fill them out and
send them into the company.

You will need to read ALL of this form,BUT you will only need to print and send page 2

This is only needed IF you are doing a wire transfer.If you are paying with a card, you may skip this step.
Step 4You will need a picture of your photo ID
